Personalized Acrylic Music Plaque

Personalized music plaques, now available! The QR code on the plaque will be personalized to open to the song of your choice on YouTube.  The plaques are 5X7" in size not counting the base. These plaques come with two options for the base. You can order the plaque with an acrylic base or with a lighted, color-changing LED base. There is a limited number of LED bases available!
